Visitor Open Gym Per 1 Hour

ASSUMPTION OF RISK

I understand that participation in gymnastics, tumbling, trampoline, open gym time, and related activities involves inherent risks including but not limited to sprains, fractures, dislocations, head injuries, paralysis, or even death. I acknowledge that I am voluntarily allowing myself/my child to participate in open gym time and accept all risks associated with this activity and am responsible for monitoring our child during the time they attend (similar to taking your child to a public park to play).

WAIVER AND RELEASE OF LIABILITY

In consideration for being allowed to participate in open gym activities at Wasatch Gymnastics Heber, I, on behalf of myself, my child(ren), my heirs, assigns, personal representatives, and next of kin, hereby waive, release, and discharge Wasatch Gymnastics Heber, its owners, employees, volunteers, and agents from any and all claims, demands, losses, or liabilities for injuries or damages arising out of or related to participation in open gym activities, whether caused by negligence or otherwise.

MEDICAL AUTHORIZATION

I certify that I/my child is physically capable of participating in gymnastics and related activities and has no medical condition that would prevent safe participation. In the event of injury or emergency, I authorize Wasatch Gymnastics Heber to seek medical care as deemed necessary, and I accept responsibility for any resulting medical bills for my own child(ren).

RULES AND BEHAVIOR

I understand that open gym is unstructured time and may or may not be supervised by a certified coach. I/my child agree(s) to follow all posted rules, behave respectfully, and use equipment safely. Failure to follow rules may result in removal from the facility without a refund.

PHOTO/VIDEO RELEASE

Paying for open gym in any form (credit card, cash, check, Venmo, and all other forms of payment) constitutes permission for us to film and or audio record your stay while attending our gym for the safety of all parties.

ACKNOWLEDGEMENT AND SIGNATURE

By paying for this class, in any/all payment forms, I confirm that I have read this agreement in full, understand its contents, and agree to be bound by it voluntarily.
