Visitor Private Lesson Per 1 Hour

ASSUMPTION OF RISK

I understand that participation in gymnastics, tumbling, trampoline, and related private instruction involves inherent risks including but not limited to sprains, fractures, dislocations, head injuries, paralysis, or even death. I acknowledge that I am voluntarily allowing myself/my child to participate in private lessons and accept all risks associated with these activities.

WAIVER AND RELEASE OF LIABILITY

In consideration for being allowed to participate in private lessons at Wasatch Gymnastics Heber, I, on behalf of myself, my child(ren), my heirs, assigns, personal representatives, and next of kin, hereby waive, release, and discharge Wasatch Gymnastics Heber, its owners, employees, volunteers, and agents from any and all claims, demands, losses, or liabilities for injuries or damages arising out of or related to participation in gymnastics instruction, whether caused by negligence or otherwise.

MEDICAL AUTHORIZATION

I certify that I/my child is physically capable of participating in gymnastics activities and has no medical condition that would prevent safe participation. In the event of injury or emergency, I authorize Wasatch Gymnastics Heber to seek medical care as deemed necessary, and I accept responsibility for any resulting medical bills.

LESSON CONDUCT AND RULES

I understand that private lessons are structured and guided by an instructor. I/my child agree(s) to follow all directions, rules, and safety procedures. Failure to do so may result in termination of the lesson without refund.

AUDIO & VIDEO RECORDING NOTICE

For safety, training accuracy, and facility security, all private lessons are monitored by video and audio recording systems. By scheduling and paying for a private lesson, I understand and agree that I/my child(ren) will be filmed and recorded during the entire duration of their stay on the premises.

Payment for the private lesson constitutes consent to recording.

Recordings are for internal use and may be reviewed for safety, training feedback, or incident documentation. They may shared for social media advertising of our gym publicly if you consent to pay for a lesson/class with us.

ACKNOWLEDGEMENT AND SIGNATURE

By paying for this lesson/class (any/all forms of payment including but not limited to cash, card, venmo, ect.), I confirm that I have read this agreement in full, understand its contents, and agree to be bound by it voluntarily. I understand that payment for the lesson signifies my consent to the terms of this waiver, including video and audio recording authorization.
